Dear Genners,

I've gotten several questions about my earlier post to request the front and
back of the SS-5. What was on the back of the form?

Apparently they had different forms at different times. When I received
only half of the information I had expected, I contacted the SSA and was
told that I hadn't asked for the back of the form. They sent it and voila..
I received the parents names.

Hope this clarifies my original post.
