One of my great-uncles' English name was Abraham Isaac JACOBS. His Declaration of
Intention and Citizenship papers show him as Abraham Isaac JACOBS. His tombstone
reads Avraham Yitzchak ben Yakov.
Perhaps the use of Abraham Isaac or Avraham Yitzchak for adoptees, converts,
orphans, etc. is a custom, however, it does not necessarily mean that is a name
of choosing, but is the person's actual name.
I hope this answers some of the questions.
