No there is only one set of death certificates for New York City. If a person
died within the boundaries of the City the Health Department keeps the records.
Its the index on line and the microfilms are at the Municipal Archives with the
1950s and beyond still at the Health Department. The New York Public Library has a
name only index of the newer ones as well.

If you are not finding a death certificate it maybe because of the name changes you
outlined or it might be because it was improperly recorded or indexed or she died
outside the confines of New York City.
