(U.S.) USCIS Updates Genealogy Request Forms #general

The United States Citizenship and Immigration Services (USCIS) has
published new editions of Form G-1041 the Genealogy Index Search Request
(https://www.uscis.gov/g-1041) and Form G-1041A Genealogy Records Request
(https://www.uscis.gov/g-1041a).

It is suggested that you read the instructions for both forms. The
new edition date for both forms is 05/19/16. Starting 08/08/2016,
USCIS will only accept the 05/19/16 editions. Until then, you can
continue to use the 12/17/12 editions.

Form G-1041 should be used to request a search of USCIS historical
databases - not for copies of records.

Form G-1041a should be used to obtain copies of USCIS historical records. To
use this form you must have a valid USCIS File Number. If you do not have
such number then you need to use G-1041 form first to obtain the number.

There are separate filing fees for each of the forms. The current fee
amounts are listed in the respective instructions. There is a current
notice of proposed rule-making to increase the fees which was reported in an
IAJGS Records Access Alert on May 7, 2016.
