1896 Birth Certificate--only one date?! #poland

I am not sure what to make of the document below. It is a birth
certificate >from Piotrkow (north of Czestochowa), >from 1896. Unlike
the usual document >from this time period, there is only one date give
for the birth and for the day of registration. What should I make of
this? I would like to record a birth day--but should I interpret the
date as being in the old calendar or the new calendar??
