I've posted a vital record in Polish for which I need a translation.

The only things I can read in this record are the name of the
individual, Chaim Ber KNISBACHER, his profession, kupec
(merchant), and the stamped location Kolomea. Please, if possible,
write out the Polish text as well as the translation, especially in the
Notes section at the bottom (uwagi, in Polish). Thanks in advance
for any help you can provide.
