Jeremy Berger asked:
When requesting a copy of the SS-5 application, I read that you need to
provide evidence of death.
Can I do this by including a printout of an SSDI index website search
showing a person's name, Social Security number and date of death?
Or do I have to have include a copy of a death certificate?
Yes, the SSDI is sufficient proof-better for them than a death
certificate because it has the SS# of the deceased, so they know which
Sam Cohen you are talking about.
