JewishGen Discussion Group #JewishGen Polish/Russian translation needed #general

Dear all:

I received a photocopy of a document >from the Warsaw
archives per the JRI-Poland program. It appears to be in
Russian handwriting, but, on the back, there is a
typewritten note, which refers to the entry that I am
looking for. It is either in Polish or is a transliteration of
the Russian. Can anyone translate this for me? It is as
follows:

Akta stanu cywilnego wyznania mojzeszowego w Warsawie
z lat 1858-1925, nr zesp. 200/I, akt zgonu na nazwisko
Chaim Mendel Tiszlerman, nr aktu 493, karta 124, sygn. 93.

The first z in the 5th word has a dot over it.

Thank you.
