I recently requested a copy of a relative's SS-5. The document that I received was not the SS-5 but a form labeled "Request for W-R Action". This document has no genealogical information. As best that I can deduce >from the document and a perusal of the Social Security Administration's web site is that when my relative applied for benefits in 1952 his entire file, including the SS-5, was sent to a Brooklyn field office where it was held for a few years and then destroyed. Does anyone know if my understanding is correct or whether the SS-5 might still exist?
