Beyond an error, without knowing any of the people involved, there
is yet one more possibility. I am extrapolating >from my memory of a Bar
Mitzvah in a (modern) Orthodox synagogue, where a young man was called
to the Torah using his maternal grandfather's name. He was the son of a
Jewish woman and a non-Jewish man, and therefore halachically Jewish.
His father's name could not be used because his father was not Jewish.
I do not know where this tombstone was or anything about the
families involved, it could be simply an error, it could be that his
father died young and his grandfather raised him and no-one even
remembered his father's name or it could be that the decedent was Jewish
but his father was not.
