As of 2013 there is a new procedure for ordering photocopies online

from the LDS (takes about 3 weeks). Basically you need the certificate

number and the mormon microfilm number. For NYC this is especially
easy as Steve Morse has a front end query that delivers both!

Step one: find the film number http://www.stevemorse.org/vital/filmnotes.html

Step two: order the cert >from the LDS....
