Zero Polish language skills I would appreciate their translations.

the full names of their parents, the date (and time, if mentioned)
of the document, the date (and time, if mentioned) of the marriage,
the names of the witnesses and the name of the rabbi who officiated
the marriage.

I would like to know the full name of the child in this birth certificate
(Akt 3, at the top of the page), the full names of the parents, the full
