This is our first posting to the SA SIG, and is an appeal for help.

We have a document (source unknown) relating to a death in the Cape Province
in 1905, entitled:
Death Notice (Pursuant to the Provisions contained in Section 9, Ordinance
No. 104). As against the normal Death Certificate, with its limited
information, this Death Notice has a wealth of additional genealogical
information, including the deceased’s birthplace, occupation, place and date
of death, names of surviving spouse and previous spouses, and names of all
children (and their spouses, if any). The document was to be sent to the
Master of the Supreme Court in Cape Town, or the Resident Magistrate of the
District.

We have two queries:
a) Was this type of document, apparently a legal requirement, limited to the
Cape Province?
b) Where are these documents stored, and how can they be retrieved? If they
are in the State Archives, what is the official name of the archives, and
its address?
