Name change in case of serious illness.

An additional name "Chaim" was added onto
a family member's name when he was seriously ill
as a child.
He lived a good 70 years after that. This does not
appear on his birth certificate which came >from Lithuania,
as he became ill as a child in South Africa. (I would
presume that in Religious Records, the additional
name must be listed through Synagogue records.)
His Death Certificate and Memorial Stone
carries the additional name, which he used together
with his given birth name, right through his lifetime.
