Translation request--GERMAN or POLISH--Viewmate 86449

Joseph Walder #650192 I've posted a record for which I need a translation. There appear to be both German and Polish versions. The document is called a "Protokoll" in German, a "Protokoł" in Polish, and appears to be something like a registration form for an occupant of the Łódź ghetto.
