Явился лично ветхозаветный Израиль-Юда Хенц, 36-и лет, портной, проживающий в посаде Шренск и в присутствии свидетелей-ветхозаветных: Пинхуса Болеславского, 73-х лет, лавочника и Давида Аппельбаума, 45-и лет, учителя, оба - жители посада Шренск и предъявили нам младенца женского пола, объявляя, что она родилась 11-го августа сего года в 2 часа дня от законной жены Шейны, урожденной Хершкович, 33-х лет.  Младенцу этому дано имя Хая-Блюма. Акт сей объявляющему и свидетелям прочитан и ими и нами подписан.

The Old Testament Israel-Yuda Henz, 36 years old, appeared personally, a tailor living in the settlement of Shrensk and in the presence of Old Testament witnesses: Pinhus Boleslavsky, 73 years old, a shopkeeper and David Appelbaum, 45 years old, teacher, both residents of the settlement Shrensk and showed us a female baby, announcing that she was born on August 11 of this year at 2 pm from Sheina's legal wife, nee Hershkovich, 33 years old. This baby was given the name Chaya-Bluma. This act was read to the announcer and to the witnesses, and they and we signed.
