'Reb' is an honorific; it does not mean rabbi.

It is highly unusual for a child to have the same name as his father.  The most common way that this can happen is if the father died before the son was born.

The exact date of death is hard to read.  The 11th of Sivan, 5722, began at sunset on June 12th, 1962, and ended at sunset on the 13th.  If the date was the first, then the dates would be June 2/3, and if the date was the 21st:  June 22/23.
