Thank you to all who answered my posting. Thank you in particular to Russ Maurer, who found the link to the "envelope" for this passport application.

This link is to 47 (!) pages of supporting documentation for this passport application. It is in Lithuanian. I would be very willing to pay for a translation of the parts of genealogical interest.

Russ noted that while the application is from 1929, there was a notation in 1940, which makes me wonder if this passport was used during the war.

I have attached a few pages as an example. Thank you for your help and suggestions!
