Is the death record the only source you have for the father’s name?

What’s the name engraved on his tombstone? It possible that the father’s name was unknown to whoever filled the death record, so they used the same name to fill the blank. Another possibility could be that a son filled the death record, and where he was supposed to write the father’s name, he wrote his own father’s name, (even if he realized his mistake, he might have left it like that, as people didn’t find the accuracy of the certificates to be crucial.
