How to include a middle or maiden name on a form that #general

Many forms these days are scanned and items written in are not captured.
When forms are not designed the way we genealogists would like and one
wants to ensure that a middle or maiden name is captured, try including
the middle or maiden name as part of the first name. Usually there are
enough spaces to get it all in. Since there are double first names out
there (ex. Mary Kay) systems are usually designed to accept a space
between two parts of a first name. It's a trade-off -- maybe someone
will think that this name was actually a double first name but on the
other hand at least all the names are recorded for posterity.

I am searching for the descendants of Sliome and Mendel Lazarow from
Radviliskis. They came to their uncle E. Lazarov in Klerksdorp, SA, in
December, 1928. Sliome was 18 and born in 1910 and Mendel was 19 and born
in 1909. The name may also have been spelled Lazarus.
