I sent away for 4 SS-5's in July. Within a month, I received 3 of
them. The last one has not arrived as yet and I think my request must
have gotten lost somewhere. I mailed the 4 requests in 4 separate
envelopes with 4 separate letters and 4 separate checks to: SSA, Office
of Central Records Operations, Attention: FOIA Workgroup, PO Box 17772,
300 N. Greene St., Baltimore, MD 21290.

I am having difficulty locating a telephone number so I can find out
where the missing SS-5 is. I called the 800 information number, but
they were unable to find a phone number for that office.

Does anyone have the phone number?
