NY Death Certificate #general

The Bronx death index shows a Paul Stiller (a resident of the Bronx)
who died August 7th 1945. His death certificate number is 30Y.
I have been told that the "Y" indicates he died outside of the 5
boroughs. Does this indicate "somewhere in NY State" or could it
indicate outside of the State of New York.
My great-grandmother's death certificate was also 30Y as listed on
a NYC death index for 10 Aug 1948. I later found out that she had
died in the Catskills while there for the summer, was subsequently
buried in the family plot in Mt. Judah Cemetery, Queens, NY.

The key for this notation is given in the front of the city index
for a given year.
