If I had looked a bit further I could have seen that what I read as an 'i' was in fact a 't' which makes it a Matrikelschein, in translation  'a registration form/card'.

I translated the German text. The hand-written parts I could not really get sharp enough but I hope (and assume) that you may be able to get it better from the original.

I've posted a record for which I need a translation. There appear to be both German and Polish versions. The document is called a "Protokoll" in German, a "Protokoł" in Polish, and appears to be something like a registration form for an occupant of the Łódź ghetto. It is on ViewMate at the following address:
