2.  hair color
3.  the notes for Question #29, regarding physical disabilities

Question 29: I can't make out the first word (maybe toupee?), but the second word is wig. The box around the words indicates it is connected to the question above.

Occupation is notions.  It looks like the haircolor says 'bold' but could possibly be 'black', with the final K going below the line.

My interpretation:

Notions (works for himself.)
Bald
Brown Wig

You can check on his occupation by looking at census records, and old city directories.

It looks like he may show up in 1919 St. Louis directory, at same address as on draft card, as Samuel MITCHELL, listed under:
confectioners (retail.)

There is a Samuel MITZEL in 1910 directory (different address), occuption:  tin.
