Could anyone translate the Hebrew writing in the attached image of the gravestone of my GGG Grandfather, Morris Lyons?

died Tuesday 11 Tishri 
(5)645 by the short count (abbreviated)
may his soul be bound in the bonds of (everlasting) life (abbreviated)

11 Tishri is the day after Yom Kippur. 

the short count eliminates the thousands place (which I added)
