According to family lore, my late wife’s ancestors changed the surname from Mendez to Mendelovich around 1766-1776 when he accepted a position as chief Rabbi of Byten the family was in Kobryn and moved to Slonim.

Surnames did not come in until c1820 in Poland. People used patronyms at the time you mentioned e.g. Benjamin son of Jacob. Mendelovich means son of Mendel. Mendez sounds like a Sephardic type name.
