The perfect place to ask for such translations is via the wonderful ViewMate feature of jewishgen.  You can upload an image of this document, mark the category as "translation-German", include a request and comments, and one of the volunteers with experience in German documents will post a translation.  As soon as that happens, you will receive an email with the results.  Here are two links that will tell you everything you need to know:
