A note about the date of death:

Rather than being given in terms of month-date, it is given in terms of the holiday with which it coincided.

The holiday of "Hoshana Rabbah" is the 7th day of Sukkot (which is the holiday of "Booths" that occurs two weeks after Rosh Hashanah).  The precise Hebrew date is the 21st day of the month of Tishri.
