Died on the 20th day of the Month of Iyar 5700
The sixth line from the top  are the initials of a verse "May his/her soul be bound in the bundle of life"
direct quote from the Book of the Prophet Samuel / Shmuel A, 25: 29, that is traditionally engraved on Jewish tombstones.
Reb is used as a form of Mr. it does not necessarily mean he was a Rabbi.
