Guidance on how to locate revision document

My first time posting here. I recently came across the attached revision list information on JewishGen’s Ukrainian database. I believe it might be my great grandmother’s family – Molly (Malka) Tcherneshefsky. With a name like that, you can imagine the number of ways it is spelled!
My request to the group is guidance on how to locate the actual document.

[Replying to the group in case it can help other folks] Many files from the DAKO archives are listed on Wikisource "Fond" 1, "opus" 325 is linked For "case" 203, you can see that the file is linked right there, as well as an index of names (that specifies I think frame number, not sure if this is the same as page # or page # in the linked PDF).
