4th line: Rubin, who died [on] the 2nd of Shevat
5th line: in the year 5652, according to the abbreviated date (abbr.)
6th line: may her soul be bound up in everlasting life (abbr.)
I am not 100% sure about what I read as Rubin in the 4th line.
