This is a very old Page of Testimony but I think nr. 9 which is ‘residence during the war’ reads Valkavishk.
Nr. 2 which asks for ‘first name’ also looks like Valkavishk. Perhaps the person who filled in the Page made a mistake by writing the place of residence before the war instead of the private name Kathryn Berman, Jerusalem |
